چکیده

Background Catheter-based atrial fibrillation (AF) therapy often uses cryo-balloon ablation of pulmonary vein (PV) ostia to achieve rhythm control. Prior studies suggest the ability of late gadolinium enhancement cardiac MRI (LGE-MRI) to visualize radiofrequency-induced scar. The optimal time after therapy to visualize cryo-ablation lesions is unknown. This study evaluates the relationship between the time of LGE-MRI acquisition after ablation and visualization of cryoballoon-induced scar. Methods Eleven consecutive patients (9 M, age 61 ± 6y) undergoing cryoablation for AF were prospectively enrolled. All patients had pre-procedural left atrial angiography with LGE-MRI (1.5T Signa HDx, General Electric, ) to define left atrial and pulmonary vein (PV) anatomy. All patients underwent repeat LGE-MRI between 7-28 days postablation. Images were visually assessed for scar (estimated as percent circumferential enhancement) detected in the PV antrum and placed in quartiles of definite LGE, likely LGE, unlikely LGE, and no LGE.

Left atrial scar burden determined by delayed enhancement cardiac magnetic resonance at post radiofrequency ablation: association with atrial fibrillation recurrence

Background Left atrial (LA) radiofrequency (RF) ablation has become routine treatment for atrial fibrillation (AF) but still suffers from AF recurrence requiring a repeat procedure. LA-RF ablation success rates vary between 53% and 85%. Delayed-enhancement Cardiac Magnetic Resonance (DE-CMR) can be used to noninvasively visualize LA hyperenhancement (scar). We have utilized DECMR to quantify LA...

A multi-center trial of LGE-MRI of the left atrium

Background Atrial fibrillation (AF) is the most common rhythm disturbance. Radio-frequency ablation (RFA) of the left atrium (LA) is effective for drug refractory AF patients. Outcome of RFA procedure depends on the degree of pre-ablation LA fibrosis and amount of post-ablation scar. Late gadolinium enhancement (LGE) imaging can detect fibrosis and visualize scar. However, only few centers with...

Visually-guided balloon catheter ablation of atrial fibrillation: experimental feasibility and first-in-human multicenter clinical outcome.

BACKGROUND Electric isolation of the pulmonary veins (PVs) can successfully treat patients with paroxysmal atrial fibrillation. However, it remains technically challenging to identify the left atrial-PV junction and sequentially position the ablation catheter in a point-by-point contiguous fashion to isolate the PVs. In this study, a novel endoscopic ablation system was used to directly visuali...

hp-GRAPPA for Delayed Enhanced Imaging of the Left Atrium

Introduction: Radio frequency (RF) ablation of the left atrium (LA) and pulmonary vein (PV) ostia has become a clinically acceptable therapy for atrial fibrillation (AF) [1,2]. Recently, a delayed enhancement MRI (DE-MRI) technique has been proposed to visualize and assess the extent of post-ablation scar [3]. This technique visualizes scar in the posterior wall of the LA and the PV ostia where...

Dependence of contrast to noise ratio between ablation scar and other tissues on patient heart rate and flip angle for late gadolinium enhancement imaging of the left atrium

Background Radiofrequency (RF) ablation of the left atrium (LA) and pulmonary vein ostia has become a clinically acceptable therapy for atrial fibrillation (AF). High-resolution 3D Late Gadolinium Enhancement (LGE) imaging can detect pre-ablation re-modeling of the LA wall and also visualize post-ablation scar in AF patients treated using RF ablation. The 3D Inversion Recovery prepared Gradient...
